The Table of Contents does not correspond accurately with headings used in the text. Footnotes are generally located below the relevant paragraphs; however, footnotes that are cross-referenced multiple times have not been duplicated. An error in footnote numbering was corrected silently, and a missing footnote [120] was inserted after being identified in the original source. Over-sized tables have been compressed and where appropriate a key has been inserted to assist the reader.
